Elite Health and Performance offers convenient virtual healthcare for a variety of common illnesses and health concerns. Telehealth visits are for patients age 5 years and older. Please be aware that not all conditions can be evaluated via Telehealth and we may need to refer you to a higher level of care. 

Conditions treated include but may not be limited to the following:

  • Upper respiratory illness 

  • Covid symptoms

  • Cold/Flu

  • Sinus complaints

  • Ear complaints

  • Pink eye

  • Acne 

  • Female UTI bladder infections (18-55 years old only)

  • Yeast infections

  • STIs 

  • Cold sores 

  • Skin rashes (excluding rashes of the eye and STDs)

  • Minor injuries 

In person visits can be arranged when appropriate for the following: 

  • Sport physicals for patients age 11-18 years old

  • Some minor procedures 

FMLA, workman’s comp, and MIQ forms cannot be completed via Telehealth 

Price 

All Telehealth visits are self pay - $50
